When to use this worksheet
Use this checklist to collect disposition and sentence-completion records before researching whether any sealing or expungement remedy exists in your jurisdiction.
Fields to record
- Final judgment/disposition
- Sentencing order
- Proof of completed probation/classes/payments if applicable
- Current criminal-history record if counsel recommends it
- State/court eligibility instructions
- Prior sealing/expungement orders if any
Questions to take to counsel
- What remedy is actually available: sealing, set-aside, expungement, or none?
- Does the final disposition qualify?
- Are waiting periods or sentence-completion rules satisfied?
- What records remain visible after relief?
Keep the source document
Write down the source of each date or fact and keep the original notice, report, receipt, or agency confirmation. A worksheet is an organizer, not a substitute for a filing, court appearance, or legal advice.
