  • Footwear or documented medical/physical issues relevant to counsel
  • Officer training records if lawfully obtained
  • Statements attributed to the driver

Questions to take to counsel

  • Which test was actually administered?
  • Do the report and video describe the same instructions and observations?
  • What local rules govern how the evidence may be used?
  • Are medical or physical records relevant to interpreting what was observed?

Write down the source of each date or fact and keep the original notice, report, receipt, or agency confirmation. A worksheet is an organizer, not a substitute for a filing, court appearance, or legal advice.
